Used for land or sea crossings to Canada, Mexico, Bermuda and the Caribbean region.
The Card is not valid for air travel, or for re-entrance into the United States from any other foreign country.
Passport Card is good for:
10 years for adults; 5 years for minors
Fees for first time applicants:
$30.00 for each adult
$15.00 for minors (under age 16)
(This fee MUST be paid with a check or money order
made payable to the U S Department of State)
Clerk's Fee for each Passport Card - $35.00
Passport Photo $10.00

Passport Book
Used for travel by air, sea or land to all destinations.
A U.S. Passport Book is valid for all forms of travel and is required to re-gain entrance into the United States from any country by air.
Passport Book is good for:
10 years for adults; 5 years for minors
Fees for first time applicants:
$130.00 for each adult
$100.00 for each minor (under age 16)

made payable to the U S Department of State)

4 Easy Steps To Apply For A Passport Book or a Passport Card
1 - Stop by the Clerk's office to pick up an application OR go to travel.state.gov to complete and print your application. (Do not sign the application)
2 - Obtain a Passport Picture (The Clerk's office will take your picture for a nominal fee of $10.00)
3 - Each Applicant MUST appear in person and bring to the Clerk's office the completed application, a Certified Copy of your Birth Certificate, your Driver's License and the appropriate fees as shown below.
Anyone under the age of 16 must have both parents present to sign their application.
Anyone 16 or 17 must have one parent present to sign their application.
NOTE: If you were born in the State of Ohio, a Certified Copy of your Birth Certificate
may be obtained at the Highland County Board of Health Department
4 - The Clerk's Office will complete the process and submit the application to the Passport Agency
